Here's how you know

These are the official datasets used on Medicare.gov provided by the Centers for Medicare & Medicaid Services. These datasets allow you to compare the average level of a nursing home's performance in certain areas of care for Medicare-certified skilled nursing facilities and nursing homes nationwide.

  • July 2025: Skilled nursing facility data has been updated for July 2025.
  • April 2025: Skilled nursing facility data has been refreshed for April 2025.
  • July 2024: CMS has posted two new nursing home guides: one for choosing a nursing home and one for living in a nursing home. Additionally, CMS updated the staffing level case-mix adjustment methodology, to accommodate changes to the Minimum Data Set. See QSO-23-21-NH for more details.
  • April 2024: CMS is updating the staffing level case-mix adjustment methodology and replacing some of the Quality Measures used on Medicare.gov, in order to accommodate changes to the Minimum Data Set. Also, to incentivize providers to submit accurate staffing data, CMS is revising the staffing rating methodology so providers that fail to submit staffing data or submit erroneous data receive the lowest score possible for corresponding staffing turnover measures.   • December 2021: Data archive files are built at the end of the calendar month using the latest available data posted at that time. The data refresh that occurred in early December 2021 used data from November 2021. This means:
    • The archive file nursing_homes_including_rehab_services_12_2021.zip contains data from November of 2021.
    • Because the November data were posted in early December, the November archive file (nursing_homes_including_rehab_services_11_2021.zip) contains data from October 2021.

        The next refresh was completed January 2022. 

  • The archive page has been rearranged such that there is now one zip file for each month, containing all files from that month.
