    • We are unable to expedite applications. During our busy season, it is simply not possible to process the high volume of applications in an orderly manner when nearly everyone wants expedited treatment. To be fair, applications are processed in the order received. If you would like the status of your application, please log in to your LVIS profile and view the date your application was submitted, then go to the LVIS portal home page to view what dates we are currently processing.

       

      To avoid unnecessary delay, please be sure your application includes all required materials when you submit it. If your application is returned for missing materials, it will go back into the queue on the date we receive the missing materials from you, and not based on the payment date of the application.

    • 2 - Renewals, Conversions, and Professionalization 
    • What are the requirements for me to renew my license? 
    • What type of License do you have?

      1. Do you have a 5-year or 10-year license?
        1. Do you have a teacher, school counselor, or an administrator license? You can renew (2) ways:
          1. 90 Professional Growth Plan (PGP) Points:

    • My PGP has been approved; how do I pay for my application? 
    • Renewing or converting your license is a two-step process. The Professional Growth Plan (PGP) is the first step to renewing, converting, or professionalizing your license.

      If your PGP has been approved, but you have not yet finished the application portion of the process, you will need to either start or finish your application. Please click this link to go to website for instructions: Current Educators. Then select either Renew a License, Conversion, or Professionalize & Renew. 

    • Access Indiana 
    • Access Indiana is the State of Indiana’s single sign-on portal. Citizens can have one username and password and log into multiple online services. This is new to LVIS.

      Please click this User Setup Guide to open step-by-step directions on how to create a login for Access Indiana and/or merging your pre-existing information with your new login credentials. 


      The process to merge an account can take 2-3 business days as this is done by our office. This is not an automated process. If you have submitted a request, please do not submit a ticket about the status of your merge. You will receive an automated email once your account has been merged. If you have recieved an email that your account is being merged, and you are still not able to access your account after 2-3 business days, then please contact our office by submitting a ticket below, and make sure that your profile is complete in LVIS.

    • When trying to renew my license, the system is saying, "no approved PGP plan." 
    • Your PGP has not been completed, submitted, and/or verified/approved. Log into your LVIS account, click "Professional Growth Plan" on the right-hand side.

      1. VIEW your PGP; if the status is IN PROCESS, please complete and submit your PGP for verification/approval.

      2. CREATE a new PGP if you do not have a PGP currently IN PROCESS.
    • My renewal Application is "In Process", how do I pay? 
      1. Log into your LVIS account.

      2. Select the Application Status link on the right-hand side.

      3. Edit the application with a status of "In Process."

      4. Walk through the application, upload all required documents, confirm, and submit the application.

      5. Once the application has been submitted, select the option to pay.
    • 3 - Additions to Existing Instructional License 
    • How do I add a content area to my license? 
      1. Are you adding any of the following?
        • Early Childhood Education
        • Elementary Education
        • English as a New Language
        • Exceptional Needs (Mild Intervention, Intense Intervention, Blind and Low Vision, Deaf and Hard of Hearing)
        • Fine Arts content areas (Visual Arts, Vocal and General Music, Instrumental and General Music, and Theater Arts)
        • High Ability

    • 4 - Ed-ID / SPN numbers 
    • What is Ed-ID? 
        • Ed-ID is a term that refers to Student Test Numbers (STN) and School Personnel Numbers (SPN) and the DOE’s system that issues, stores, and tracks those identifiers. It is not a newly required or additional identification number.
    • Updating school and LVIS teacher data 
    • If an educator’s email is not correct and the educator has an LVIS account, the educator must log in to their LVIS account and follow the prompts to update their LVIS email. An LVIS account email address is expected to be a personal email address, and email changes must be made by the LVIS account holder.

      If the educator’s name, SSN, or date of birth is not correct and the educator has an LVIS account, the educator must contact the Educator Licensing office to make corrections using this jotform. The educator is required to provide verification/documentation of the information being changed.

    • 2 - Emergency Permits 
    • How do I apply for an emergency permit?

    • I’m trying to apply for a new/renew emergency permit, but I don’t see that option. 
    • You can’t apply for a new emergency permit or renew an emergency permit until July 1.  The application option will not populate in LVIS until July 1.

    • How long does it take for an emergency permit to process? 
    • Once you complete and pay for the application it will route to your school employer for verification. Once verified, the application will route back to IDOE for processing. Processing usually takes approximately 10 to 14 business days from the time the application is routed to DOE. This time may increase during our busy time of year (July 1 – October 1).

    • I’m checking the status of my emergency permit. 
    • To check the status of your emergency permit application, you will log into your LVIS account and select the Application Status link on the right-hand side.

    • What do I need to renew my emergency permit? 
    • A condition to renewing an Emergency Permit is to be working toward full licensure in the content area of the permit. Progress must be completed after the issue date of the emergency permit being renewed. Official proof of progress may include:

      1. Coursework documented with official transcripts.

      2. Multiple attempts at required licensure test(s) documented by official test scores.

      3. Verification from a college or university (signed letter on school letterhead) that a required college course was cancelled, resulting in a situation where the student has no control over the course offering.

      4. Program/classes/schedule needed by the student has a future start date, verified by the college or university
    • 3 - Reciprocating an Out-of-State License 
    • How do I transfer my out-of-state license? 
    • Completion of an application in the online Licensing Verification and Information System (LVIS) portal is required to determine an applicant’s eligibility for licensure. Apply for an Original Instructional license as an out-of-state graduate. Once an application is submitted, an evaluation of all documentation provided will be completed. Applicants will be sent the results of the evaluation in an evaluation/deficiency letter to the email address listed in their LVIS account.

    • What is Verification of Experience? 
    • If you have prior teaching experience, please upload a letter from your employer verifying your years of experience. The letter must be on letterhead and physically signed, OR refer to the link below to download the Verification of Experience Form:

    • 4 - Alternative Licensure Paths 
    • I have a bachelor’s degree (or higher degree) that is not in education, and I would like to become a teacher. How do I become a licensed teacher?

    • 5 - CPR/Suicide Prevention 
    • Can CPR be taken online? 
    • You can complete a blended course (an online classroom component followed by an in-person hands-on component) or you can do it all in-person. A class offered totally online WILL NOT be accepted.

    • What type of CPR training should I complete? 
    • A class offered totally online WILL NOT be accepted, nor will a training with a remote evaluation.

      The CPR certification must contain the following: 

      • Cardiopulmonary resuscitation that includes a test demonstration on a mannequin.
      • Removing a foreign body causing an obstruction in an airway, and the Heimlich Maneuver;
      • Beginning July 1, 2011, the use of an automated external defibrillator (AED); and
      • Hold a valid certification in each of these procedures from either the American Red Cross or the American Heart Association or other providers as approved by the department.

      The CPR training can be either adult, pediatric, or blended adult and pediatric.

    • Where can I schedule a CPR class? 
    • Please refer to the link below to obtain the list of our approved CPR providers. You can schedule a course through one of the providers:

    • Where do I go to complete suicide prevention training? 
    • You can complete a suicide prevention course through one of our approved providers listed below. The course MUST be at least 1-hour long.

    • I have already completed a suicide prevention training; does it meet the requirements? 
    • If the training is at least one clock hour (60 minutes) in length, it should meet the IDOE requirements.

    • I completed my CPR and/or Suicide Prevention training as promised in my out-of-state application. Where do I turn it in? 
    • You can submit your CPR and/or Suicide Prevention training certificates to transcript@doe.in.gov or email them to the evaluation specialist that completed your evaluation. Their contact information will be in the evaluation letter you received when you obtained your license.

    • 6 - Substitute Permits 
    • I would like to apply for a substitute permit.

    • How do I renew my substitute permit? 
    • There are no renewal applications for substitute permits. You must apply for a new permit. Please see the following link for step-by-step directions:

    • 7 - Username/Password Reset 
    • How do I obtain my username and reset my password? 
    • To obtain your username or reset your password, please click on the link below:

      You will be required to enter your CURRENT email address, date of birth, and the last SEVEN digits of your social security number.

    • How do I change my name in LVIS? My name has legally been changed and I cannot do it in LVIS.

    • I have never logged into LVIS, but it is saying that I already have an account linked to my SSN. What does that mean? 
    • LVIS was created in 2011 and all information on licenses that predates that was transferred electronically into the system. The system recognizes your name and social security number, but it does not recognize your email or other information since you have not yet set up an account. To set up your account in the most secure way possible, please call the educator licensing help desk line so a member of the Educator Licensing team can help you set up your account over the phone: (317) 232-9010.

    • 8 - License not Populating 
    • License not Populating 
    • If your license doesn't populate (isn’t available to select for an action) while completing your application, please complete the email section below (in this JotForm).

      If your license status says 'SPN Review' or 'Post-Coding Processing' there was an error with your license being generated through the SPN system that is linked with ED-ID. This is due to some of the following errors:

      • Did you put dashes in your SSN when making your LVIS 360 account? These dashes cause errors.  
      • You might have given your school your SSN but put your Driver's License Number instead in your LVIS360 account profile. If you did, then it will not match in ED-ID which generates your SPN number. If this is the case it will throw an error. 
      • Your SSN might be incorrect. In the profile section of LVIS you are only able to see the last four digits for security reasons. If you believe it could be incorrect, please see the solution below:

      Please contact us by calling our helpdesk at 317-232-9010 to have us correct any issues with your LVIS profile where you cannot edit yourself. Or, you can fill out a name change request on the left-hand side. Please put in the notes you are not changing your name, rather updating your SSN and upload a copy of your social security card. 

    • 9 - Documents sent in to be uploaded/Documents not uploading 
    • My required documents are not uploading in the application. 
      1. Are they too big?
        1. If the file is over 5 MB, try one of the following:
          1. Use a PDF compression tool such as www.smallpdf.com.
          2. Try optimizing or reducing the size of the PDF in Adobe Acrobat.
          3. Try printing your document as a PDF and rescanning it.

      2. Are they the right file type?
        1. The system will only attach the following file types: pdf, txt, doc, docx, jpg, xls, xlsx, and ppt.

      3. I have tried both of the above, and it is still not working.
        1. Your school's firewall system may be categorizing LVIS incorrectly. Have your IT team unblock the URL: https://gov.content.powerapps.us  This usually will resolve the issue  

           

        2. If you are using Chrome, Safari, or Firefox; try using Internet Explorer. LVIS is a Windows-based website and runs best on Internet Explorer.

           

        3. If you are using a Mac, tablet, or mobile device, try using a PC or Windows computer. LVIS will run better on a Windows device.

           

        4. If you have tried the above troubleshooting options and still cannot attach them, you can send the documents to transcript@doe.in.gov. In the application, when prompted to upload your document, please upload a Word document instead stating you are emailing your documents to our office.

    • 10 - License Status 
    • When does my license expire? 
    • You can view the status of a license at any time. License information is public data and can be accessed through the public educator license search without even having to log into your LVIS.

    • What does my application status mean? 
      1. Advisor Material Review – The application is currently being reviewed by the appropriate License Advisor at the recommending institution/institution where you completed your preparation program. All in-state preparation programs process initial licenses for their program completers. Please see the following page for their contact information:

      2. Approved – The application has been approved and the license has been created. It is available to view/print. Click on the link to View/Print license on the right side of the screen when you log in to your LVIS account. You will be able to download it as a PDF to save to your computer, email or print.

      3. Closed – The application remained idle in the status of ‘In Process’ for 45 days. At that time, the system automatically closed the application. This means your application was not submitted. It can also mean your application remained idle too long in Waiting on Documentation without a response from you.

      4. In Process – This is the first stage of the application process before the applicant has submitted the application. An application “In process” is incomplete, but it can be completed at any time before the passage of 45 days. If it sits idle for 45 days, It will close automatically.

      5. Legal Review – The application will be reviewed for a second time based on criminal history questions/answers.

      6. Pending Application Payment – The applicant has submitted the application but has not completed the payment process. Applications are not reviewed until the payment process has been completed. Please go to Pay Pending Applications on the right side of the screen when you log in to your LVIS account to view your pending application and to make the payment.

      7. DOE Material Review – Once the application has been submitted, a review is conducted of the license requirements and materials you submitted by an advisor at the DOE. This process might take 2-4 weeks, especially in the busy summer months.

      8. Pending Evaluation Payment – Certain applications require an Evaluation Fee to pay for an evaluation to determine if the applicant is eligible for a license. In this case, a second payment is required to issue the license you are found eligible for. If an evaluation is conducted, you will receive an evaluation letter from a Licensing specialist listing any conditions or deficiencies.

      9. School Material Review – The application is currently being reviewed by the appropriate school corporation personnel where you are employed. Please contact your school corporation if you have questions or concerns regarding this process.

      10. Unable to Approve – The applicant did not meet certain requirements/guidelines. This was determined during the application review process.

      11. Vocational Director Material Review – The application is currently being reviewed by the appropriate Career and Technical Education (CTE) director. Please contact your CTE director with concerns regarding this step in the application process.

        If you do not have a CTE director or you are not seeking a CTE license, you may have completed the wrong type of application. In this case, please contact the helpdesk to correct your application.

      12. Waiting on Documentation – The application was reviewed, and additional documentation or information is needed from the applicant. An email has been sent to the email address in the applicant’s LVIS account requesting the additional information.

        If you do not know what documentation has been requested or cannot find the email, please click the link for E-mail Log on the right side of the screen when you log in to your LVIS account. You will find all the email correspondence that has been sent to you.

      13. Ready for Evaluation – The application has passed Material Review and is ready to be evaluated. Please be patient as an evaluator is reviewing your application. Check your email for updates regarding your evaluation.

      If your status is any other status that the ones listed above, please contact us using the form below as there might have been an error when your license was generated.

    • 10a - What can I teach with my license 
    • How do I know what I can teach with my license? 
      1. Go to View/Print License in your LVIS account to view your license information. Then, you will want to compare your license information to the assignment codes for Indiana. You can find the assignment codes by going to: https://www.in.gov/doe/educators/educator-licensing/what-can-i-teach-with-my-license/

      2. I want to add a new instructional area to my license.
        • Please go to the additions tab of this form in the top drop-down to see how to add an additional area to your license.
    • 11 - Professional Growth Plan (PGP) 
    • What are Professional Growth Points? 
    • Professional Growth Points are claimed for completion of professional development (PD) activities and recorded in a Professional Growth Plan (PGP). A PGP is one of two (2) methods to renew a professional educator license. A PGP is the record of one’s professional growth/development using the PGP tool found in your LVIS account. In addition to school-based PD, educators can claim PGP points for attending conferences and workshops, participating in committee work and school improvement plans, as well as taking coursework relevant to teaching and education. Many of these activities can be included in one’s PGP. One hour (60 minutes) of PD equals one PGP point unless the activity has been pre-approved for a higher number of points. One semester hour of college credit equals 15 PGP points.

    • When can I start recording PGP Points? 
    • PGP points must be completed since the issue date of the license being renewed. For the third (3rd) renewal of an Initial Practitioner (IP) two-year license, the PGP points may be obtained since the issue date of the Original IP license.

    • Where can I find PGP opportunities? 
    • You can find PD opportunities on the IDOE Moodle site: https://moodle.doe.in.gov/. The IDOE also keeps a list of PD opportunities and updates it periodically. You can find the list here:

    • What does my PGP status mean? 
      1. In Process - the PGP has not been submitted. Applicants can continue to enter PGP points into this PGP.

      2. School Review - the PGP has been submitted to the selected school employer. The administrator at the employing school will verify/approve the PGP. Questions and concerns about your submitted PGP should be directed to the administrator at the school or corporation.

      3. IDOE Material Review - the PGP has been sent to the IDOE for verification/approval. IDOE will verify the submitted PGP. IDOE verifies/approves PGPs for educators not currently employed in an Indiana school.

      4. Approved - the PGP has been verified/approved. An application to convert, renew, and/or professionalize using the PGP may be completed. To Professionalize and Renew one of the following 90-point PGP Types is required: Normal License Renewal or Renewal of a 5-or 10-year license.

      5. In Process Returned - the submitted PGP was returned to the applicant for changes/corrections.

      6. Unable to Approve - the PGP cannot be approved. Additional documentation may be required. This has been communicated to the educator by email with the subject: Office of Educator Licensing - re: PGP returned.

    • My PGP was approved, why did my license not renew? 
    • When renewing with a Professional Growth Plan (PGP), there are two (2) required processes:

      1. Entering PGP points into LVIS and submitting them for verification. Steps 1-16 on instructions.
      2. 2. Completing the application. This process cannot be completed until the applicant has received an email indicating the PGP has been verified. Steps 17-31 on instructions. Completing these steps is how the application is submitted and required documents are uploaded (i.e.: CPR/AED/Heimlich cards). Once submitted the application fee can be paid.

    • What is a PGPEI? 
    • A PGPEI is a PGP for educational interpreters (EIs) to renew an educational interpreter permit. A PGPEI requires 36 points, with 18 points required in skill development and 18 points distributed in other selected areas. For more information on renewing an EI Permit:

    • PGP Expiration 
    • Once verified/approved, a PGP will have an expiration date five years from the date it is approved. If the PGP is In Process however, there is no “shelf life” for that PGP.

    • How do I submit my PGP for verification? 
      1. I am employed by an Indiana school:
        1. Once the required PGP points have been entered into the PGP for verification, a box will appear at the bottom of your PGP. Click the radio button: Employed by an Indiana School. Select the school/special education co-op/corporation where you are employed and then Submit PGP for Verification. The PGP will route to the administrator responsible for verification/approval. Contact your administrator regarding any questions.

      2. I am not currently employed by an Indiana school:
        1. Once the required PGP points have been entered into the PGP for verification, a box will appear at the bottom of your PGP. Click the radio button: Other. The PGP will route to the IDOE for verification/approval.
    • How can I edit my PGP? 
    • A PGP with the status of In Process or In Process Returned can be edited. On the right-hand side of the PGP table there will be a blue link that says: View. Click on View to edit your PGP.

      If you make a mistake on a PGP entry, you cannot edit the entry. The entry must be removed and re-entered.

    • 12 - Special Education Alternative Route License 
    • What are the requirements? 
      1. Bachelor’s degree from a regionally accredited post-secondary institution.

      2. Verification of enrollment and an established program of study in an approved alternative route program for special education licensure.

      3. Verification of employment in an Indiana school as a teacher in the special education content area that corresponds to the approved program in which the applicant is enrolled.

      4. The application must be approved by the school employer.

      5. The school employer may partner with the approved alternative route licensure program where the license holder is enrolled to ensure the teacher:
        1. Receives high-quality professional development that is sustained, intensive, and classroom-focused to have a positive and lasting impact on classroom instruction before and while teaching.

        2. Participates in a program of intensive supervision that consists of structured guidance and regular ongoing support of teachers or a teacher mentoring program.

      1. For educators licensed in another subject: Currently licensed teachers who wish to add special education to their existing license can participate in a fully funded 18-credit hour graduate program through the University of Indianapolis, Taylor University, or Indiana Wesleyan University.

      2. For individuals currently enrolled in a special education preparation program: Individuals who are currently enrolled in a special education teacher preparation program can earn scholarship assistance to support the completion of their required special education coursework and purchase textbooks.

      3. For non-education bachelor’s degree holders: Special educators who have not earned full licensure but hold a non-education bachelor’s degree can receive a full scholarship to enroll in a federally-compliant Transition to Teaching program.       4. For individuals who have not yet passed the state licensing exam: Individuals who have completed a special education preparation program but have not yet passed the state licensure exam can participate in a free test-preparation workshop through IndianaSEAL.

    • Waiting on Documentation – The application was reviewed by a member of our team, and additional documentation or information is needed from the applicant. An email has been sent to the email address in the applicant’s LVIS account requesting the additional information. Please check your spam folder if you are missing this email.

      If you do not know what documentation has been requested or cannot find the email, please log into your LVIS account and go to 'Missing Documents' in the left-hand menu. You can then view all the active additional documentation requests, and are able to click on the blue links there to open them and view the message that was sent by our team with a description of either the missing document or other information on your application.

      1. I am currently outside of the United States or US Territory:
        1. There are firewalls in place that do not allow our system to be accessed outside of the US or US Territories. You can do one of the following:
          1. Call a friend/family member that is inside the US who you trust and have them help access LVIS for you or use a trustworthy VPN to access the website.

          2. Call our licensing help desk between the hours of 8:00 AM - 4:30 PM EST Monday - Friday (except national holidays). We may be able to assist you to gain access.

      2. I am currently inside the United States or US Territory.


        A. The site could be down for maintenance and we have the site in a private mode while we test to make sure the system is running correctly before coming back online.         B. If you are on a school issued device or on school wifi your district's IT could have a firewall in place preventing LVIS from working correctly. Please contact your school's IT department with any issues with your browser or issues downloading or uploading documents. Your school's filter system may be categorizing LVIS incorrectly. Have your IT team unblock the URL: https://gov.content.powerapps.us This usually will resolve the issue.

        C. There is an error that says 'Failed to Fetch': Your school's filter system may be categorizing LVIS incorrectly. Have your IT team unblock the URL: https://gov.content.powerapps.us This usually will resolve the issue.

        D. You might need to clear your internet browser's cache. The directions for this is different depending on what internet browser you are using (Edge, Chrome, Firefox, etc.) Directions for this can be found by a Google search.

    • 18 - Verification of Experience Letters 
    • Many applications require a letter verifying  experience. What is this? 
    • This is a signed letter from your supervisor or HR officer on school letterhead stating the position you are in and the number of years of experience you have served in your role (teacher, administrator, counselor, etc). The letter should be physically signed.  It can either list your dates of employment or years or service.       Items that are NOT acceptable:

      1. teaching contracts

      3. job offer letters
      4. a copy of your license

    • Verification of experience for Administrative Conversions or Professionalization 
    • For Conversions, you must be an Assistant Principal or Principal. For any other role titles that are used (i.e. Dean, Chief, etc) – your letter needs to be specific as to your role and how it aligns to that of a traditional Principal or Assistant Principal role.

      For Professionalization – you must have 5 years in your role for the specific license type (i.e. Building Level Administrator, Director or Superintendent) and have 60 + graduate credit hours from an accredited institution. Graduate credit hours are hours earned in a Master’s, Specialist or Doctoral program (above and beyond undergraduate, bachelor degree credits).

    • All fees are non-refundable. Please do no make duplicate payments, or pay for mulitple applications of the same type. 

    • Trouble with Payment 
    • If you have made a payment and your application still states it is waiting on payment, DO NOT MAKE A SECOND PAYMENT. Fill out a request ticket below, and indicate the type of application and the date it was paid, and the name on the credit card that paid for the application. An associate will review your payment and add it to your application as needed.

       

      If your application was an evaluation for any of the following types: All Out-of-state, Non-US graduates, Communication Disoders, Advanced Degree, and Career Specialist Permits will require two payments. One for the evaluation and one for the actual license if you are found eligible. If your evaluation letter, which was sent to you via your email, states you were eligible, it will contain information on this second payment.

       

      Please make sure you are not using a school issued device or on school Wi-Fi. Also, do not use Safari or Firefox internet browsers, or cell phones. These options may lead to issues with making payments and uploading documents.

    • Transfer a Payment 
    • If you have paid for an incorrect application, payments can be transferred to the correct application if within 90 days. If longer than 90 days, then a new payment will need to be made.

      Please do not make a second payment if you think you have filled out the wrong type of application, instead please notify us in a ticket below, and we will inform you if your payment is eligible to be transferred.
