The Oregon Department of Consumer and Business Services wants to hear your stories on prescription drug prices.

Please use the form below to share your story about how prescription drug prices and medication costs have affected you. All consumer stories provided by Oregonians are included in the annual report to the Oregon Legislature and published on our website: dfr.oregon.gov/drugtransparency/Pages/annual-reports.aspx.

Each year, the department holds a public hearing on prescription drug prices. We will read consumer stories at the hearing, and we encourage you to attend and provide testimony. Email rx.prices@dcbs.oregon.gov if you have questions about the public hearing.

We encourage you to also report increases in your prescription drug costs to the department: dfr.oregon.gov/rxdrugprices. Let us know if you need help reporting a price increase.

This survey can be provided upon request in an alternate format for people with disabilities or in another language. For questions about accessibility or to request an accommodation, email rx.prices@dcbs.oregon.gov or leave a message at 503-947-7200 (or toll-free at 833-210-4560).

Some of these stories will also be shared at the State of Oregon hearing on prescription drug prices, which is held each year in either November or December. View hearing details at dfr.oregon.gov/drugtransparency/Pages/public-hearings.aspx. We will not share your name or any personally identifiable information at the hearing or in our report.
