When CMS prepared to disburse payments, we were unable to verify banking information for the QPs listed in the Excel spreadsheet in this zip file. We need your assistance in verifying banking information. This zip file includes the Excel spreadsheet and a PDF of the APM Incentive Payment Notice and 2025 Billing Collection form.
