I-693, Report of Immigration Medical Examination and Vaccination Record
If you are applying to adjust your status to lawful permanent resident (also known as getting a Green Card), use this form to establish that you are not inadmissible to the United States on health-related grounds.
Effective Dec. 2, 2024, you must submit Form I-693, or a partial Form I-693 (such as the Vaccination Record), and you must submit it with your Form I-485. Otherwise, we may reject your Form I-485.
The following aliens applying for adjustment of status must submit a partial Form I-693 (Parts 1.-5., Part 7, and Part 10, Vaccination Record) when they file their adjustment application:
- Refugees (Form I-590 or Form I-730) who already completed an immigration medical examination conducted by a panel physician outside the United States; or
- Derivative Asylees (Form I-730) who already completed an immigration medical examination conducted by a panel physician outside the United States and applied for adjustment of status within 1 year of eligibility to file.
The following aliens applying for adjustment of status must submit a partial Form I-693 (Parts 1.-5., Part 7, and Part 10, Vaccination Record) when they file their adjustment application if their vaccination record was not properly completed and included as part of the immigration medical examination conducted by a panel physician outside the United States:
- Aliens admitted to the United States as a spouse, or child of a spouse, of a U.S. citizen or fiancé(e) or child of a fiancé(e) of a U.S. citizen (K-1/K-2/K-3/K-4 Nonimmigrant) who already completed an immigration medical examination conducted by a panel physician outside the United States no more than 1 year before they file to adjust their status; or
- Afghan nationals who arrived in the United States under Operation Allies Welcome who already completed an immigration medical examination conducted by a panel physician outside the United States.
NOTE: If you received an immigration medical examination outside the United States and the results reveal a Class A medical condition, or if there is reason to believe you have acquired a Class A medical condition after admission or parole into the United States, you may need to complete a new immigration medical examination. You should review the Form I-693 Instructions to determine what information on Form I-693 you need to complete and submit to USCIS, if required.

If you complete and print this form to mail it, make sure that the form edition date and page numbers are visible at the bottom of all pages and that all pages are from the same form edition. If any of the form’s pages are missing or are from a different form edition, we may reject your form.

After completing your immigration medical examination, the civil surgeon must give you the completed Form I-693 in a sealed envelope. Do not accept it if it is not in a sealed envelope. We will return the form to you if it is not in a sealed envelope, or if the envelope has been opened or altered.
You (not the civil surgeon) must submit your Form I-693 to USCIS. If you are applying for adjustment of status to lawful permanent resident (a Green Card), you must submit Form I-693 by mail with your Form I-485, to the location specified for your Form I-485 (see the Direct Filing Addresses for Form I-485). Otherwise, we may reject your Form I-485.
Other aliens applying for adjustment of status: Follow the instructions on or included with the application, or the instructions given to you by the office requesting the immigration medical examination.
If you are filing Form I-485, and if you are required to submit a Form I-693 or partial Form I-693 (such as the Vaccination Record), you must submit it with your Form I-485. Otherwise, we may reject your Form I-485.

eMedical
The Centers for Disease Control and Prevention (CDC) updated the Tuberculosis Technical Instructions for Civil Surgeons. Civil surgeons must register for access to eMedical, an electronic health processing system. Using eMedical will help the CDC maintain public health security and transition to electronic submission of immigration medical examinations. Civil surgeons should have received emailed instructions to register; it is important that you follow these instructions and register immediately. If you did not receive any emails with instructions for registering for access to eMedical, please email OPSCivilSurgeons@uscis.dhs.gov.
