The Derbyshire Home from Hospital Service

The Derbyshire Home from Hospital Service provides practical support for vulnerable people leaving hospital, or those living at home but at risk of being admitted. The Service is for people living in Derbyshire (outside of Derby City) that need support. Support can include:

Prescription and shopping collection
Providing temporary key safes
Arranging pendant alarms
Referrals and signposting to other services
Support with letters/paying bills
Emotional support
